Y8 HPV Vaccination - Consent

During the summer term vaccinations will be taking place for Year 8 students
Students will receive their 1 st HPV (Human Papilloma Vaccine) vaccination in Year 8 and their 2nd HPV vaccination in Year 9; the same consent form is used for the 1st and 2nd doses of the HPV vaccination.
